VitaMedical - Independent Licensed Nursing Support

VitaMedical connects families with independent, licensed nursing professionals who may provide in-home or remote clinical support based on individual needs and physician guidance.

These providers operate independently and are not employees of Vita Care or Everguard Home Care. All nursing services are delivered directly between the client and the licensed provider.

Understanding the Types of Care Providers

At Vita Care Referral Agency, we help families connect with independent care professionals based on their individual needs. Each provider type serves a different role within the care journey.

All care services are delivered directly by independent providers. 

  • Hands-on personal care (bathing, grooming, dressing)

  • Assistance with mobility and transfers

  • Support with activities of daily living (ADLs)

  • Basic comfort and safety monitoring

  • Assistance in home or facility care environments

Certified Nursing Assistants (CNA)

  • Vital sign monitoring (blood pressure, pulse, temperature)

  • Basic nursing care and documentation

  • Assistance with care plans as directed by a provider

  • Post-acute or ongoing nursing support

  • Patient condition monitoring within scope

Licensed Vocational Nurses (LVN)

  • Skilled nursing assessments (when ordered)

  • Wound care and dressing changes

  • Post-hospital or transitional care support

  • Medication administration

(if applicable/permitted)

  • Clinical monitoring/reporting

Registered Nurses (RN)

Vita Care does not provide medical or nursing services and does not supervise clinical care.

How It Works

Families who may require additional clinical support are introduced to independent registered nurses who can provide services such as:

  • Skilled nursing support (as permitted by license)

  • Health monitoring and assessments (when ordered by a physician)

  • Post-hospital or transitional care support

  • In-home nursing services 

All clinical services are determined, directed, and provided by the licensed nurse or supervising medical provider.
