Before You Begin Registration

To ensure your registration process goes as smoothly and efficiently as possible, please have the following documents ready to upload before starting your registration process:

• Government-issued photo ID (Drivers License, Passport, etc)

• Proof of COVID-19 vaccination (if applicable)

• Current Live Scan or Background Check

  • If you don’t already have one, you may complete a personal background check through Checkr.

• Recent TB Test results (if applicable; not required)

• Proof of Liability Insurance

  • We recommend obtaining coverage through CM&F Group

    (Select: Individual → Home Health Care → Caregiver/Companion → Apply Now)

• Proof of Vehicle Insurance (for client transportation purposes — if you do not drive, please specify)
